I don't divide by gender because I have some unisex items, and I have some nonbinary sims. Within each, I mostly divide by slot type, but I do have PJ's as it's own folder in each age folder because some child and toddler separates sets I have came merged.
With the CAS folder, I take full advantage of the 5 subfolder limit. All you have to do is click on the file in question and then on the two arrows at the top of the application window. I create new folders for item types as needed and subdivide when it's clear I can subdivide them.
